Individuals may make nominations for one person or a group of caregivers.

The name, phone or email of the nominee OR a point of contact for the group that you are nominating

A short essay describing the impact they have made in your life

By submitting an essay, each nominator authorizes GTD to post the essay (or portions of the essay) along with the nominator and nominee(s) name(s) to the website.

Two lunches a month will be donated. Dates will be coordinated with each recipient.

A maximum amount of 10 lunches will be donated for each selected recipient.

Photos of each luncheon will be shared on the GTD website, Facebook and Instagram.
